Clearly there is another alternative to the dilemma outlined in the NYT article. Direct Care Practices can provide the PCP with a high level of professional satisfaction, while also solving the financial stability and quality of life issues.

I agree that the concierge movement can have an important impact on primary care. In contrast to most who support this model, I am in favor of much of the recent health reform legislation. I do think we need more equity within our system, as well as some of the other elements of the legislation. I don't think that means that all patients should be required to receive care from physician members of large hospital based group practices. I believe that there is room for flexibility to accomodate both types of practices, but it seems clear that relying on insurance based revenue as a private practice primary care physician has become difficult to sustain. Still, patients may seek out smaller more personalized practices. I hope that insurers will explore options that make it affordable for patients who prefer these types of practices--perhaps with lower cost, high deductible plans and HSA's.

Roy,

After watching the bill pass tonight, I did strat to wonder in earnest whatwill happen to my and my wifes HSA . We each have separate BCBS Carefirst indiv 2500 deductibles with HSAs.

I will be coming o NY in April for meeting so I look forward to yourpresenation. I am a new member, just starting my Concierge practice , I am well established since 2003 with many FFS programs , I have both an insurnace based practice and a FFS practice which provides weight, bioidentical hormones , functional medicine , antiaging prograsm all ffs . I am trying to integrate them all . I feel the 'private medicine" component using my other prograsm as attraction will be my best bet to Obama care.

Just hope this government does not outlaw private medicien in the short rn as I suspect many MDs will not take the ne rates which , along with the red tape, are likely to reduce MD income.

I am hopeful that the passage of the Bill will promote patients into lookng to secure a spot in a good medical practice.
